8/26/2023 EnchantaVaughn Approved Behavior: Client was seen standing at the front door when his counselor arrived. Client waved and hugged QMHP as he invited everyone in. Behavior: Client was able to cope and relax in his environment and complete his assignment. Client communicated positive attitude and engaged in activity without frustration or being distracted by external stimuli. Client gave really good feedback and was responsive to all instructions given to him. Intervention: QMHP praised client for his positive conduct and waiting quietly for the next directions. QMHP complimented client on his outfit, clean face, and fresh haircut and display of good personal hygiene. QMHP supported client with completing his chores and helping his mom cook, clean, and straighten up the living room and kitchen. QMHP applauded client for the behavior and manners that he was displaying at this time. QMHP coached client to acknowledge and list his strengths, resilience and the positive outcomes that often occur when he makes informed decisions opposed to impulsive ones. QMHP processed client positive behaviors as he went into the next activity. Intervention: QMHP reviewed several strategies for conflict resolution and positive social behaviors with client in efforts to increase clientâs awareness. QMHP explored positive self-talk, use instruction modeling, and increase social communication skills with positive reinforcement role play activities. QMHP shared with client that this is the type of behavior his mom had tried to encourage him to display last year. QMHP verbalized the awareness that all people are unique, learn differently, and have various strengths and weaknesses. QMHP reviewed the positive things that client had done and encouraged him to continue in making positive decisions in all settings. Response: Client said he woke up feeling good and was ready to have a good day. Client expressed positive thoughts about himself. Client verbalized positive feelings about goals and objectives, confirming his commitment to achieving them. Client displayed good participation and was responsive to all instructions, interacting well with mom and QMHP. Client responded in a positive manner acknowledging the positive reinforcement that the QMHP provided. Client responded well to feedback and discussed some things he can do better so that he is always ahead of himself and know what to do if has a minor setback. Response: Client acknowledged interventions and verbalized clear understanding of each intervention. Client displayed positive self-talk, ability to follow instructions, role play of appropriate way to act at home, increased communication and exhibiting the positive reinforcers that were provided by QMHP. Client was able to process making good choices and avoid making bad ones. Client has implemented the correct behaviors, by avoiding disruptive and impulsive behaviors around his family and this allowed everyone to move in their own way and have their own space. Client was able to demonstrate fairness and kindness throughout the session. Therefore, Mom was in compliance of the next in-home visit.
